We are committed to delivering the highest quality care at a reasonable cost. Payment is due at the time services are rendered unless prior arrangements have been made. If you have any questions regarding your account, please contact our office at 850-425-1300. In many cases, a simple phone call can quickly resolve any concerns. Please note that you are fully responsible for all fees charged by this office, regardless of your insurance coverage.

We will provide you with a monthly statement. Most insurance companies respond within 4 to 6 weeks. If your statement does not reflect your insurance payment within that timeframe, please contact our office. Any remaining balance after insurance has processed is the patient’s responsibility, and prompt payment is appreciated.
